The Social Care Network (SCN) is a New York State program that connects Medicaid members and their families with nutrition support, housing assistance, and transportation. If one member of your household qualifies, your entire family may be eligible to receive meals at no cost.
Enrolled in Medicaid Managed Care in NYC
Pregnant or a new parent (up to 12 months postpartum)
A frequent hospital or emergency room visitor
Enrolled in a Health Home program
Managing a mental health condition, substance use disorder, or developmental disability
Recently released from incarceration (within 90 days) with an ongoing health condition
The SCN also connects eligible members with housing assistance, transportation to medical appointments, and a dedicated Social Care Navigator to help coordinate services.
Before you apply:
Participating will not affect your Medicaid benefits. You may opt out at any time.
Services can be renewed if your need continues beyond the initial period.
If you are pregnant or have children, additional food may be provided for your family.
Your information is only shared with your explicit consent to coordinate services.
